Whats with pre med students shadowing a doctor

Many medical schools are either requiring or highly recommending that applicants show evidence of “shadowing” [following a doctor around] for varying periods of time. This supposedly gives a pre-med student an idea of what doctors do. I guess the schools assume that if someone has shadowed a doctor and still wants to become one, that individual is a better candidate for medical school than someone who hasnt done any shadowing.

A recent incident at a hospital in Syracuse, New York raised some serious concerns about shadowing. An anesthesiologist allowed a college student to endotracheally intubate a patient in the operating room. This was a problem on many levels. Students who are shadowing are not supposed to touch or examine patients. The patient who was intubated likely did not know that an unlicensed college student would be doing a procedure on him. And of course, theres HIPAA.

According to the article, the director of Consumers Unions Safe Patient Project, called the incident an "egregious violation of patient-doctor trust."

Ive had a problem with shadowing for many years, and Im not the first to say so. Dr. Elizabeth Kitsis, director of bioethics education at Albert Einstein College of Medicine in New York, has blogged about the topic.

She told of a male pre-med student who was introduced to patients as a "student doctor" and watched a gynecologist perform pelvic exams. The student himself said he felt a little awkward. One wonders how the unsuspecting patients would have felt had it been known he was a college student thinking about becoming a doctor.

There were many comments pro and con on both Dr. Kitsiss blog and a follow-up piece that appeared on another Einstein blog.

Dr. Kitsis co-authored a paper which found that few studies have looked at shadowing by pre-med students. She called for guidelines and a code of conduct for this activity.

Several questions come to mind.

With all the information available on the Internet, is shadowing really an effective way for college students to decide whether to become physicians or not?

Is there any research comparing career outcomes of pre-med students who shadowed doctors to those who did not?

What about the patients? Do they have any say in this? Are students who shadow introduced as who they really are?

How does a student choose a doctor to shadow? As far as I can tell, there is no quality control for this aspect of shadowing.

Is shadowing mandatory in other fields? Must one shadow before becoming an engineer [civil, railroad, or sanitation], an accountant, a fighter pilot, a shepherd, or an exotic dancer?

What to do with abnormal PSA results in a young man

A 45-year-old man in excellent health with no family history of prostate cancer had a screening PSA done three years ago which was in the range of 4.0 ng/mL. He has been followed by a urologist, and the test was repeated several times without much change.

In June of this year, his PSA was 4.6 and the free PSA was 0.6 for a ratio of 0.13. He was given a course of antibiotics for presumed prostatitis, and repeat testing a month later showed a PSA of 3.8 with a free PSA of 0.5. Because the PSA was less than 4, a ratio was not calculated.

The patient obtained copies of the reports. The from June one states the following: "When total PSA is in the range of 4.0-10.0 ng/mL, a free PSA/total PSA ratio of less than or equal to 0.10 indicates a 49% to 65% risk of prostate cancer depending on age. A free PSA/total PSA ratio of greater than 0.25 indicates a 9% to 16% risk of prostate cancer depending on age." It does not comment on the significance of a ratio of 0.13, which I have looked up. The cancer risk is in the area of 20%-25%. However, no source gave estimates for men under the age of 50.

On examination, his urologist can feel no nodules. He has recommended that the patient undergo an MRI of the prostate.

Stating that an MRI is not indicated in a man of his age with his history, the patients insurance company will not pay for the test and suggested a trans-rectal ultrasound. The urologist advised the patient not to have an ultrasound due to his age and the potential for complications. A hospital quoted him an out-of-pocket price of $2500 for the MRI.

The urologist has told him that random biopsies may not be accurate and there is a risk of complications.

When he had his first PSA done three years ago, I had expressed surprise and wondered why it had been offered to him. The patient said his internist told him he should have the test.

He is concerned about these recent results and has asked me for advice which I am not qualified to give.

Numerous urologists responded on Twitter with more than 60 tweets about this post. Suggestions for the next step were as follows: trans-rectal ultrasound (TRUS) and biopsy as mentioned by Dr. Cooperberg below; going ahead with the MRI; repeating the PSA in 3 months; go for a second opinion by a recognized expert in prostate cancer.

One urologist emailed me to point out that even if the patient has cancer, it is probably not an aggressive type because his PSA has not risen in 3 years.

Another urologist gave me the name of an expert in the patients geographic area. 

The patient has already scheduled an appointment for a second opinion.

Working with RSS Feeds

Okay, so now what you want to do is go to Google, and search for any website that posts information and updates about the specific niche that you chose to base your twitter account on.

When you find the site you want, look for something on the page that will link you to that websites RSS feed, If you have trouble finding if they provide feeds or not, you can either go to "FIND" or "SEARCH" located in your browsers toolbar and type RSS in the search box, or enter the web site address in Google and add rss after it.

eg: gamestop.com rss

This will usually bring you to a page where you can select what rss feed you want, some sites offer multiple rss feeds so select the one you feel fits your needs.

Adding Your RSS Feed To TwitterFeed

Go back to twitterfeed.com, and create a new feed.

Add Feed To Twitterfeed

For this step all you have to do is copy the url of the feed you want to use, and paste it in to twitterfeed.

Add whatever you want for the name, and in the "Blog URL or RSS Feed URL" area, copy/paste (add) the url of the rss feed you want to use.

Advanced Settings

On the same screen click the "Advanced Settings" button and go to "shorten link through", there is a drop down box beside it, open it up and scroll to the bottom and select "Custom", Leave it empty for now and go back to Adf.ly, while logged in to adf.ly go to the top of the page and click Tools, on the tools page go to "API Documentation" and click.

On this page you will see a link that looks like this http://api.adf.ly/api.php?key=################################&uid=######&advert_type=int&domain=adf.ly&url=http://somewebsite.com

# represents your id and key.

Now take your key and uid and place it in this link (replace the # with your key and uid)

http://somewebsite.com Must Be Replaced With %@ In order for this to work.

Also you may have noticed that in the modified link "advert_type=int" has been removed, this is because TwitterFeed has issues showing the interstitial ad (full page ad) and just leads to a dead page when clicked, or shows the ads but doesnt redirect to the desired page, so the only way to fix this is by removing the "advert_type=int" instruction from the link and adf.ly will automatically use the interstitial link instruction and will work properly with TwitterFeed.

Now when thats done, all you have to do is copy and paste your new link into the twitterfeed Custom endpoint box and "Continue to step 2" on twitterfeed.

"There are more advanced options that will serve you well, but you can play around with that on your own time."

LAST STEP!!!

Make sure that you are logged in to the twitter account that you want to use with twitterfeed, you are going to want to activate your twitter account with twitterfeeds oauth so it will automatically post feed/blog updates to your twitter account using the rss feed that you provided to twitterfeed, when thats all done, click done at the bottom, twitter will now get updated feed posts from twitterfeed, and the links in each post will now be automatically shrunk using adf.ly and posted to your twitter updates, and every time someone clicks your adf.ly link, you make a bit of cash.

It wasnt like this in my med school

When I was a medical student, we had to practice drawing blood on our lab partners. I remember the first day we did it. One guy fainted as he was having his blood drawn, and another fainted while he was drawing someone elses blood.

Weve made a lot of progress in medical education since then. In 2015, teaching blood drawing, which is going to eventually be taken over by robots anyway, is passé.

Students are suing a Florida sonography school because they were forced to perform transvaginal ultrasounds on each other almost every week. Those who complained were allegedly told to “find another school if they did not wish to be probed” said an article in the Washington Post.

While that seems out of line, it pales in comparison to allegations lodged against a former US Army doctor who ran a company that taught battlefield medicine to soldiers and made more than $10.5 million in the process.

According to Reuters, he gave students alcohol and drugs, including ketamine, a powerful hypnotic used as an anesthetic. Sometimes alcohol and ketamine were given at the same time.

Trainees were told to insert urinary catheters into each other, and two students underwent penile nerve blocks. On another occasion, when students balked at receiving penile blocks, the doctor had the students perform a penile nerve block on him. Its not clear what a penile nerve block has to do with treating wartime casualties.

If thats not troubling enough, he supposedly ran what he called "shock labs," during which he drew blood from trainees, observed them, and gave their blood back to them.

But wait, theres more. The doctor is alleged to have had a few beers with a student and examined, manipulated, and photographed the students uncircumcised penis.

The doctors claim that his methods are standard in Virginia medical schools was refuted by experts quoted in the Reuters piece.

The Virginia Medical Board has suspended the doctors license and will hold a hearing on June 19.

And we thought sticking each other with needles was traumatic.

Interview Sculptor ReevzFX

 I am a great admirer of sculptor ReevzFX work. His highly detailed stunning pieces has made him one of the most sought after commissioners in the world of costuming. Reevz took some time to chat with the Confessions of a Cosplay Girl Blog about his craft and his thoughts on Hollywoods take on Superhero costumes.


Victoria: How long have you been sculpting?

Reevz: About 7 years. I tried sculpting in high school but was more of a comic book artist. The more time passed, I connected with industry pros online and figured Id try my hand at sculpting masks and Ive been doing it ever since.

Victoria: How long does it take to create a cowl?

Reevz: Depends on the design and the deadline. Normally I block everything out in a a few hours, then I spend about a day or so getting the forms even and work on the symmetry. Then I spend another few days detailing and surface cleaning. The sculpting process goes pretty fast if its something Im doing for myself but most times a client will change the design along the way which adds more time to the final sculpting process. I then seal the sculpt, make the mold and once that is finished I am able to produce the castings. The whole process from sculpt to casting normally takes about 3-4 weeks.

Victoria: Whats the most difficult cowl or accessory youve created?

Reevz:  The new Wolverine cowl was pretty tricky. The mold needed to be done a certain way to keep the castings seamless. Also casting then in 2 colors is pretty laborious.

Victoria: Whats your favorite piece you have made?

Reevz: Probably my ManBat mask. It was one of those commissions that came out better than I had envisioned. I was sad when I had to ship it out to the customer. I wanted to keep it ;)

Victoria: Is there anything you would like to make that you havent created all ready?

Reevz:  I have a whole list of personal projects that I would love to tackle at some point. The Thundercats characters being the big one. Its just a matter of getting some down time to get it done.

Victoria: How can people contact you for commissions?

Victoria: What are some of your favorite cowls and props that youve seen in Hollywood films?

Reevz: The 1989 Batman suit is my ultimate favorite (as most people who know me probably already know ). The Dark Knight batsuit is a close second. I think that the suits in The Watchmen are brilliantly executed . Ive seen them up close and they are flawless. Jose Fernandez is one of THE names in super hero suits in Hollywood so usually anytime his name is attached to something I always know itll be a work of art.

Victoria: What are some of the worst superhero costumes youve seen on film?

Reevz: I really dont like to shit on anyones work, but I wasnt a big fan Halle Berry Catwoman costume. Just wasnt my cup of tea.

Victoria: Poor Aquaman seems to be the butt of all the jokes in the Justice League. Why does he get picked on so much?

Matt: The JLA look at him as not being able to do much with whatever powers he has. Hes an easy target, so the Justice League targets him. hahaha.

Victoria:  Have you been reading the New 52 and if so, what are your thoughts?

Matt: I have been reading a few. Liking what Im reading.

True grit

In case you havent noticed, a hot new topic in education is "grit." In order to reduce the long-standing 20% attrition rate of surgical residents, some say we should select applicants who have more grit or conscientiousness.

A recent paper in Surgery reported residents who dropped out of programs had decreased levels of grit as measured by a short-form survey. But due to unexpectedly low attrition rates in the surgical programs participating in the research, the study was underpowered to show a statistically significant difference in outcomes of high-vs. low-grit individuals.

Its hard to argue with the premise that choosing applicants with high reserves of grit might lead to better retention and performance of residents.

I blogged about this three years ago in a post called "Harvard says train residents and medical students like Navy SEALs."

Unfortunately, identifying who has grit will be much more difficult than simply testing those applying for surgical residency training.

Below is the eight-item grit survey, which is scored on a 1 to 5 Likert scale.

1. New ideas and projects sometimes distract me from previous ones
2. Setbacks dont discourage me
3. I have been obsessed with a certain idea or project for a short time but later lost interest
4. I am a hard worker
5. I often set a goal but later choose to pursue a different one
6. I have difficulty maintaining my focus on projects that take more than a few months to complete
7. I finish whatever I begin
8. I am diligent

The survey is rather easy to "game." You can take it online and see for yourself. Figuring out how to achieve a high grit score should be obvious. My grit score was 5 making me grittier than 90% of the US population.

You can avoid any uncertainty about passing the test by reading the paper which conveniently spells out which answers result in a high grit score. If the word ever got out that applicants to surgery residencies are being screened for grit, you can bet that only those with high "clueless" levels will score poorly. At least the clueless would be weeded out.

Attempting to assess grit in an interview would be equally hard for the same reason. Picture this conversation.

Program Director: Do you finish what you start?
Applicant: No, I dont.
Program Director: Are you a hard worker?
Applicant: No, Im not.

Who is going to answer grit-related questions in a non-gritty way?

Heres another issue.

Despite rigorous background checks and thorough testing of physical and mental qualifications, some people selected for Navy SEAL or astronaut training fail to make it through.

Compare that to the process of selecting surgery residents, which involves sifting through several hundred applications for an average of four positions per year. Medical school grades and USMLE scores dont predict resident performance or success, nor do deans letters and letters of recommendation from mentors.

Most programs conduct two or three interviews of 40 or 50 candidates for a total of maybe 45 minutes. Even if one luckily identifies a gritty applicant, she may not end up in the program. Because of the way the matching algorithm favors the applicants choices, a program will almost never end up with all of its top candidates anyway. Its a bit of a crapshoot.

What can we do if we match applicants with average or low grit? An article in the Huffington Post says we dont know how to teach people to be more gritty. I think its like common sense—a trait that cannot be changed or improved.

Assessing applicant grit levels might help, but the short-form grit survey is probably not the answer.

The robot is here to draw your blood

A company has produced prototype robots that can draw blood from human arms. Here is a 48 second video showing one of them in action.



Using an infrared camera, the robot identifies a suitable vein and accesses the vein with ultrasound guidance.

A second video, not embedded in this post, explains that the robot is about 83% successful at drawing blood which compares favorably to the success rate of experienced human phlebotomists. The robots inventor hopes to refine the procedure to get the success rate up to 90%.

It also says that there are 9 billion blood draws per year in the United States suggesting that a market is certainly there.

The second video also mentions the discomfort patients experience when a phlebotomist misses or damages the vein.

However, the robot needs quite a bit of help from a human phlebotomist both before and after the actual blood draw takes place.

The robot cant apply a tourniquet, cant wipe the skin with alcohol, can’t take the cover off the needle, once the vein is accessed cant actually remove the blood from the vein with a syringe, cant take off the tourniquet [the human assistant didn’t do it correctly either*], can’t apply pressure to the puncture site, cant put a Band-Aid on the skin, and can’t safely dispose of the needle.

It’s not clear what the robot would do if a patient flinches or withdraws his arm. Patients have been known to do those things.

For the foreseeable future, the robot would have to be accompanied by a human. So whatever the cost of the robot, personnel would not be eliminated.

Its a clever idea, but I don’t think a robot will be drawing your blood any time soon. ________________________________________________________

What comes after the Heimlich maneuver

At the end of an otherwise informative article about the nuances of performing a Heimlich maneuver, New York Times science reporter Jane E. Brody recommends that if all else fails, a cricothyrotomy should be attempted.

She goes on to briefly explain how the procedure is done. In the right hands, a cricothyrotomy is safer and easier to perform than a formal tracheostomy. However, for a layperson who has never seen either procedure done, does not know the relevant anatomy, and has never put a knife to anyones skin, it is highly unlikely to be successful.

Ms. Brody includes a link to website with some static drawings of the procedure. The site is called Aarons Tracheostomy Page and it bills itself as "The Internets leading tracheostomy resource since 1996."

Heres an excerpt from that description of the operation:

"3. Take the razor blade or knife and make a half-inch horizontal incision. The cut should be about half an inch deep. There should not be too much blood." Yes, there should not be too much blood, but sometimes there is.

Both the Times article and the reference repeat the medical urban legend that the barrel of a ballpoint pen can be used as a breathing tube.

A 2010 paper found that due to high resistance to airflow, most ballpoint pens are not adequate airways, and the two that were acceptable (the Baron retractable ballpoint and the BIC Soft Feel Jumbo) are unlikely to be on hand. An earlier paper also reported similar high airflow resistance with ballpoint pens.

A small study involving inexperienced junior doctors and medical students found that they were able to successfully perform cricothyrotomies in only 8 of 14 cadavers. Injuries to the thyroid and cricoid cartilages were common.

Remember these important points—cadavers dont need an airway in a hurry and they dont bleed.

Evidence of successful cricothyrotomy by bystanders is lacking. A 2010 review of American soldiers killed in Iraq between 2003 and 2006 noted that five of those who died appeared to have had attempts at cricothyrotomy, all of which failed.

I once was asked to see a patient whose "cricothyrotomy" done in an ED by an experienced emergency physician and a resident turned out to be a laryngotomy. The tube was inserted directly into the larynx.

To the uninitiated, surgery looks easy. Last year I blogged about Malcolm Gladwells outrageous claim that just about any college graduate could become a cardiac surgeon.

I suppose one might say "What have you got to lose? The patient is dying. Try the cricothyrotomy." I can’t stop you. But be certain it is necessary, and realize your chances of success are extremely low.

If you’re considering it, at least look at some of the many instructional videos available online.

Warning: Graphic. There is some blood. Here’s one by an ED doc. In a non-hospital setting, you would not have all the help and equipment he had. Here’s another, this time by a surgeon—with lots of help and equipment. Both patients were relatively thin.

Now imagine doing it with a pocket knife and a ballpoint pen on an obese person. Still think it’s easy?
